  • Direct Deposit Form

    To enroll in Full Service Direct Deposit, simply fill out this form and submit it to The Alpha Group Payroll Department at payroll@thealphagroup.com, or fax to 508-285-7116. Please also include a form or letter from your banking institution confirming your account information. Feel free to contact Payroll with any questions!

  • Paycard Form

    If you choose to have your paycheck deposited to a Pay Card, simply download the document and submit it to our payroll department. We will then contact you to determine a convenient time for you to pick up your card.
